Adoption Profile Questionnaire

We ask our fosters to please complete this questionnaire, as they know the dog better than anyone else in rescue. The information fosters provide enables the rescue to compose an accurate and meaningful “Available for Adoption” posting, so that we are sure to find the best possible forever home for the dog. Thank you for all you are doing to help us rescue these wonderful dogs!

General Personality / Demeanor

1. Tell me about the dog’s personality. How would you describe him/her to a potential adopter? (I.e.: loving, cuddly, aloof, independent, sneaky, etc.) : *

2. How does the dog react to, or interact with, other dogs? Do you think they would do well in a home with other dogs or should they be an only dog? *

3. How does the dog react to, or interact with, cats? Do you think they could they live in a home with cats? *

4. How does the dog react to meeting new people? *

5. How does the dog react to children? Do you think they could live in a home with children? If so, what age range would be appropriate? *

6. What are his/her favorite things or things to do? (i.e. walks, toys, treats, belly rubs, etc.)? *

Are there any "must haves" in a forever home for this dog? (Other than a yard. You will have a chance to talk about whether they need a yard in the Environment section.) *

7. Is there anything that should be avoided in his/her forever home? (Things/situations that make them anxious, uncomfortable, or they dislike) *

1. Is the dog house trained? Have they had any accidents or marked? *

2. Is the dog crate trained? How does he/she behave in the crate? *

4. Would you consider this dog to already have basic training and good manners, or are they in need of basic training? *

5. Have you seen any resource guarding (of food, toys, space, humans), dog reactivity, separation anxiety, or other specific behaviors or issues that adopters need to be aware of? *

Environment

1. What do you think is the ideal home environment for this dog and why? *

2. Would this dog do well in an apartment/condo with no yard? If not, please explain why. *

3. Is this dog a “bolter” or escape artist? If so, please describe the circumstances that might lead to them escape and what would be required to prevent it. *

Activity Level

1. How would you describe the dogs activity level? (i.e. highly active, moderately active, or lazy/couch potato) *

2. What would be the ideal amount, and type, of activity for this dog to be happy? *

Photos

Please upload at least 3 photos of your foster dog to be used in their “Available for Adoption” posting on the rescue’s website and social media. Please remember that the best photos are ones where the dog is looking in the direction of the camera, you can see their entire body (a cute close up is also ok, as long as we have some showing their entire body), and there are no other dogs, cats, or humans in the picture.
